Statement of Claim
Claimant says:
"My family has been trying to get my deceased fathers pharmacy records from Landmark Hospital of Joplin for months. Landmark Hospital provided us with incomplete medical records in the first place, and then refused to give us the pharmacy records after many inconsistencies were found regarding medications that were given to my father during his brief stay. (He was stable when he arrived there and then transferred to another facility after he rapidly declined. He died 3 days later.) We contacted PharmaCare, who told us to contact the director of pharmacy at Landmark, Rance Junge. We sent a request to he and another pharmacist whose signature is on some of the medication orders, Randal Morris and received no response. Gretchen Blake from corporate admitted in a letter sent to my mother, that at least one page of the Medications Administered Record was missing and could not be reproduced because it may have had documentation on it. It feels like these records went "missing" intentionally. We filed a complaint with the CIHQ and received a letter stating that Landmark Hospital was in violation of at least one regulation but they did not give us anymore information than that. We just want to know what happened to my Dad and we believe those missing records will give us that information. We need to know what happened so we can have closure."

  • 08-25-2017 — Rapid decline of my father and no accurate record of medications he was administered while in Landmark Hospital of Joplin’s care. Transferred to your facility in stable condition and rapidly declined and was transferred to another hospital where he died.
