Statement of Claim
Claimant says:
"My name is Herman Ray Jackson, birth date, 01/04/1972. Mailing Address, 1201 3rd. st. NW. Alb.NM. 87102. SSI #****4995. I lost my wallet around March/ 01/2017. On March/03/2017, went into jail, at M.D.C. in Albuquerque, N.M. My release from there was Nov/01/2017. After a month of trying to get a call to Net Spend, through my caseworker .In May/00/2017. I talked with a guy named Kars a supervisor that works at Net Spend. I advised him that i was in jail and to not send my card to any address because he told me that he could not send it to me while i was in there. i asked for him to not send my card to any address to freeze the account in tell, i was able to get out and call and send them proof of my identity. reference # 5888860922 claim # 3369117, #3749439, # 3497496. i missed a call because i was sick, from the dispute dept. Asked for a call bake and never received one. Then received a few letters saying the pin was never changed and it matched a sending habit of old transactions, basically blowing off the fact of my fax that was sent to them of my were a bouts at the time of the transactions were being placed, or the fact that i asked in May, 2017. A supervisor, named Kars. To freeze my account because i was in jail. Come to find out all my money hasn't been returned to me. I've a police report about it and just want my money back. This is my disability money. I had no way of using my money or would ever let anyone have my money while i was in jail someone took that money. (505) 582-0281, my card should of been stopped in may but it wasn't."
