Claimant says:
"I had this company apply siding to my home 4 years ago, knowing that I would receive a rebate of significance. I have sent in all my papers as directed in my 30 day window, they came back with insufficient address, unable to forward label.
I tried calling the phone numbers the company left with me and they are all out of service.
I have sent in the forms on the 14th of this month. And just got them back on Friday.
Please let me know if you have any information on their where abouts, or how I can get the correct address to send my forms, time is running out.
