Statement of Claim
Claimant says:
"I started working for the Illinois Division of Rehabilitation services on 11/5/2014. I was terminated with no cause on May 13, 2016. My father Charles Smith is my patient (client) that is able to speak for hisself. However was not asked to do so by his case worker Loretta Wright and her boss Susan Huwe that came to our resident May 13, 2016. My sister Sheinina Smith-Hicks whom I haven't seen since 2015 got mad at me and called my patients' case worker and told her to terminate me for no reason and this happened in April 2016 in which I was delayed payment that I had provided. Waukegan office phone number is 847-224-9763, which is where I drop my time cards off at and are considered a third party for the Secretary of the State of Illinois and I received a W-2 at the end of the year.
I also recieved three letters from my sister Sheinina Smith-Hicks that was hand written from California and mailed to me in regard of her harrassing me over a personal issue. Sheinina Smith-Hicks work for Landstar for the Department of Defense driving around the world and I haven't seen her until May 13, 2016. Sheinina Smith-Hicks drew up false blurry Power of attorney papers without my father/patient (Charles Smith) knowledge. Sheninia Smith-Hicks continue to call Loretta Wright my patient (Charles Smith) case manager of my father/patient to terminated me with no cause and Loretta did so on April 3, 2016 and held my paycheck for weeks. I have not harmed or neglected my Father/Patient/Client (Charles Smith) in any way. I have not recieved any notice of Shenina Smith-Hicks, Loretta Wright or Susan Huwe plotting againist my employment with the State of Illinois.
On April 2016, as I was dropping my time card off a DRS representative by the name of Delores spoke with me and told me that I was not going to get paid because Loretta Wright terminated me without my patient (Charles Smith or myself) knowledge. That same day another representative Lisa told me that my previous time cards had disappeared and she did not know what happen to them and that she would fix the issue but it will be about 2-3 weeks before I get paid. I had copies of the previous time cards and Lisa apologized to me for the misunderstanding. That same day Loretta Wright my client/patient (Charles Smith) the case worker on his case didn't speak to me that day after seeing me and later on that day I also found out that Loretta had been disclosing my paycheck information to my sister Sheinina Smith-Hicks.
I contact the State of Illinois in Springfield in April 2016 about about all that was going on with my job and I was told to continue to work by Marybeth Scholten at the main office in Sprinfield, IL which is Susan Huwe boss. Marybeth Scholten office in Springfield number is 217-782-2004.

My sister Shenina Smith-Hicks had been giving my patient/father Charles Smith caseworker Loretta Wright gifts/cash to do any and everything to degraded my character and harrass me all of a sudden. While at work on May 13, 2016 my sister Sheinina Smith-Hicks arrived from California after driving trucks for the Department of Defense cross country. I have been working with my father for 18 months from Nov 5, 2014 until May 13,2016. I am seeking loss of wages and attorney fees. I still don't have a clue to why I was terminated without proper notice.

My sister Sheninna Smith-Hicks arrived Shortly after Loretta and Mrs. Sue Heuy came to my work/house to confront me with termination without warning or cause. My sister Sheinina Smith-Hicks illegally drew up Power of attorney papers on my patient/father (Charles Smith) and he said that he did not give her permission nor did he sign such documents for Sheinina Smith-hicks to have control of my patient/client (Charles Smith) person or assets. I saw the papers and asked my father/patient/client if the documents were true to his knowledge and he answered NO. I moved from Georgia to take care of my father (Charles Smith) and there is no one at home to give him 24hr care

Loretta (case worker), Sue Huey (DRS manager) and my sister Sheinina Smith-Hicks proceeded into the dining room leaving my father/patient/client in the leaving room area with no decision making in regards to hisself (Charles Smith) the client. Later on, I was told that I was terminated with no cause and that I was being replaced by a homemaker from Waukegan Home Services at 7.5 hours a day. I providing care for 10 hours a day that included weekends if needed. My sister Sheninia Smth-Hicks is not at home to see after my father wellbeing she will be returning back on the road, I am!"
