Statement of Claim
Claimant says:
"Back on July 24th my whole family switched to T-Moble after being with Verizon for over 20 years. They had a special where they offer unlimited data for your account. They would also pay off your phone and charges. The person that worked with us was Moe. My point was we found out later on was.

Had to be a certain phone, which we were never told up front (we had all but 2 out of seven of the phones.
For you to be reimbursed the sales person had to fill out the form that day. They told me to call 611 or their customer Service. 
When I called their customer service, they said the sales person had to fill out the forns.
When we went back the sales person was no longer there.
It took five visits to get all the phones working within the system
We were told from the the start our bill would be $249.00, we knew that and we added two tablets, so the price would be a bit higher. 
We also spoke to the Retail Associate, he informed us to call 611 or their customer Service.
After we called the customer service and they sent us back to the store to fill out the forms needed. We spoke to Yesenia D. about filling out the forms to be reimbursed for the charge from Verizon. After she informed us that these forms should be filled out the day we transferred to T-Mobil and it was too late now to be reimbursed. 
Now, please understatand I am only looking to be fair. There are My son and daughter purchased a new phone and supplies. I am only looking for what's fair, all's I'm looking for is do what you say you are going to do. In this case pay for the transfer cost and pay off the phone . I have paid over $2,042.40 so far.
