Statement of Claim
Claimant says:
"Verizon has continued to bill me for an incomplete service hookup at a business location in Glen Arm, MD.

The entry point for this issue is a call placed by me to arrange for service. The call was during the day at approximately 11AM EST. The call was routed to an individual who sounded to be of East Asian or East Indian origin. The sound quality was plagued by scratchiness and static. I asked that the Verizon representative call me back. The scratchiness and static on the line continued. Previous phone calls from my line had no issues with call quality. After many attempts to properly spell out a variety of items of information. The name of my firm was still misunderstood by the representative. The name was entered incorrectly and thus incorrectly listed.

FIOS service was deemed unavailable at the business location. DSL was also unavailable at the business location and agreement was made to be put on a wait list for DSL out of the FORK, MD exchange. Two POTS service lines were to be connected. The representative stated that an appointment would be scheduled to arrange hookup of the two POTS lines.

No appointment contact was ever made.

The rented office suites the business was two occupy were fully wired for multiple extensions with a total of (7) wired wall jacks from a previous business.

Approximately, one week after the initial hook up order. All phone jacks in the offices were removed and the wiring was left in a less than workman like manner. This was during the first weeks of the first month of my occupancy of the office suite.

The landlord claims that he did not provide access to the empty offices. No business card or other paper work was left by any Verizon employee. The landlord stated that he did not provide access to the wiring closet to Verizon personnel.

Verizon began billing for the connection and sending mail to the inappropriately spelled company name that the East Indian/Asian telephone representative had taken down. This billing material was sent to my home address, looked like advertising and was thus tossed into the shredder.

I inquired with Verizon about the state of the FIOS service that was to be installed at another location - that had been placed with the same order. Evidently the contractors that FIOS had called in for that installation, did not desire to perform the additional work required for that installation. After several visits from Verizon and Miss Utility, with no installation progress and no evidence of service at the new office location - Verizon and I agreed to cancel the order for installation.

For phone service and internet the business relied upon Verizon Wireless 4G LTE. Verizon Wireless was able to provide high quality interaction, service and documentation --- though at a substantially higher cost.

However, I was still being personally billed for service to (qty 2) unterminated POTS lines in the office suite which I could not use.

The billing statements were arriving in the form of FIOS advertising and propaganda materials to my home and continued to arrive with the mangled company name. As the materials looked too flashy to be bills and as there was seemingly no service and an emailed order cancellation notice, these bills continued to be filed in the shredder unopened.

My cost for the fiasco is substantial."
