Claimant says:
"I was at the walgreens drug store on oct 14,2013.as I was going now this is the first I have ever been in this store. the store doors opened as the doors I stopped,i turned aroundas I turned I caught myself there was a ashtray by the door.

as I dropped down to keep from falling, i caught myself with my left hand, i kicked the ashtray ,and I b tried to clean it up. i looked around and there seem to be no one around. i have had problems with my left arm ever since. the insurance co.said they have sent me a denial letter. but i have not recieved it. i walked around in the store rubbing my elbow and moving my arm. i ask for help in finding a product. i was told to ask a pharmasits.i walked up to the window aand they was very unprofessional, one of them told me sir i am on the phone.i went to the next window and still had to wait. i finally got one of the cashiers or assistant to help me find the product i was looking for.i have tried to talk to the insurance co. and they are telling me there is nothing they can do after they had reviewed the tape. i went to the doctor,my doctor old me i had streathed muscle and tissue. my fingers go numb. my girlfriend does all she can to keep me from hurting.but i still hurt. now they are saying i did not fall but was walking around in the store as if nothing had happened.i have asked to see the video, but was told they could only release it to law enforcement. i don't have the money to hire a lawyer can you please tell me what i need to do. also they transfered the manager that was there to another store right after this happened .thank you melvin wolfe"
