Claimant says:
"I brought a Office Executive Chair it base came lose and I fell to my knees , injuring my knees, lower back, and Right Shoulder. I purshcased this defect office chair in 4/2009.I still have the chair but it is not in use due to it is still broke and a person can not sit in it because it tilt over and it is a danger to a person .
They refuses to take back chair, pay my lost wages, medical bills, medications, pain and suffering, transportation to physician, medical equipment, and etc."

  • 08-05-2009 — Fell out of chair because the part broken on base of chair and the chair tilted causing my fall which I fell on knees and hurt my lower back and right shoulder.
  • 08-13-2009 — Went to Physician had Xrays of both knees, lower back and right shoulder. Started on pain medication, knee brace, heating pad to lower back, starting having help with bath, cooking, laundry, meal preparation,then I was not able to work due to not able to put pressure on my legs due to pain.
  • 09-17-2009 — Notified company made demand to pay medical bills and refuses.
