Statement of Claim
Claimant says:
"I had file insurance through market place for me and my husband, it was Aetna insurance. I am also paying on time for long time and in the march of 2016 my husband got sick we went to emergency hospital in Carolina medical center in pineville North Carolina. After a month we got a bill from hospital saying that he is not covered by insurance, i called to Aetna and they said you got this insurance from market place so we cant help here you have to called them. I called Market place for my insurance problem they said who enrolled you he/she didnt do your husband name under your insurance. I said but i am paying full amount since than and now you saying he is not in my insurance? why? We had this insurance since 2014 when we moved to NC. Then they said we can try to do special enrollment for him then after a month i called again and find out that he has been declined no one called me and said this until i called them and find out whats going on. i had called them 10 times since then now they are giving excuses not to enrolled him in the insurance what can i do. I did submit documents they need it since then but still no answer back. Now they telling me to go somewhere near by to get help or appeal request form to fill out all that nonsense excuses."

  •      I have been waiting a month to receive my 1095 A form form Marketplace in London KY, I live in Ohio and this is the location that services our Marketplace Insurance. I have spoken to numerous people and was told the call was placed in escalation and was promised the form to be sent in no more than 2 weeks . This problem has been ongoing now for a month my tax return was filed however I need this form and cannot seem to get it. We moved and I have told each representative and given our new address each time and also been told I can see the 1095 form online on the website however I am unable to see the 2016 year as I have stated each time I call. I am very frustrated that I cannot seem to get this necessary form when I have been told it was being sent several times. I do not know what else to do as I asked for a supervisor the last call and it was the same thing I was told before and yet no form 1095 has arrived ! Please help I would like to receive my income tax return but it is on hold as no one can seem to send it as I have asked for many times now !
