Claimant says:
"I had business transaction with the Art Of Dentistry on July 24, 2014 where a service (if you want to call it that) was provided to me. Partial payment for the service was made ($402.50). After I decided that no further services would be provided (because of poor business record keeping and poor business procedure) and it was determined that I owed Art of Dentistry $120.00. I am due a refunds of $282.50 on a credit card and $150.00 that was paid by check.
I have had several telephone conversation with Christy (Office Manager (??)) and the issue continuously gets pushed off onto somebody else or something else.
Today is October 28, 2014 and there is still no resolve."
