Claimant says:
"The crown of my front tooth came off January 17, 2014. It had been placed in my mouth by Dr. Moen who has retied. This crown was put on about twelve 12 years ago.
I went to Shallow Dental Care January 17, 2014, and discussed the problem with Dr. Brian Wilkinson who examined the tooth location and stated that he would replace the crown. That to replace the crown would cost $1,562.00. Dr. Brian Wilkinson stated that all the dental work would have to be paid up front prior to him doing any work on my tooth.
The work/detail he stated that had to be done included the following: 02751 Crown Tooth 8
02954 Post and Core prefab for crown Tooth -root canal therapy Molar Tooth 8 -
He stated he had to kill the root - I had no pain -the root was already dead 12 years ago - prior to the crown coming off. Also that he would have to make a new crown that the one that came
off was no good - it was just a cap. I had the crown in my hand and it appeared to represent a whole tooth. So to gain a fix on my front tooth so I could smile again, my husband was asked to write a check for $1,562.00 so they could proceed. The check was issued and they proceeded to place a false cap on the remaining part of the original crown location were a peg remained.
The dentist stated that he would have to send out of the office for the new crown to be made and that I should return in a week.
I returned 2/27/2014 to have the new crown placed in my mouth. The dentist Dr. Wilkinson was working on taking off the temporary, it was very hard to get off, and broke the old tooth in several pieces, however the new crown did not in fact match my other teeth so any further work had to wait until he ordered another crown to hopefully match my other teeth. Another temporary tooth was placed in my mouth.
We waited and - returned to the dental office on 2/5/2014. He then matched the tooth and it was not a match - then he did a Root Canal Therapy Molar Tooth 8. Stated he would order another crown,to return on 2/27/2014.
2/27/2014 I returned and I was told that he could not replace the crown alone that I would have to have a bridge and it would cost another $794.00. The only other option he gave me was to give me a refund of the $1,562.00 and I would have to go to another dentist to have the mess and the carelessness he caused to be fixed by another dentist at a higher cost - estimated at $2,356.00 or more to fix the mess he caused. He stated he would make and install the bridge for a lower cost of $794.00.
So I had no other choice but to stay and pay his price to fix what he messed up.
On 2/27/14 I had a surgical Extraction, BBRG-Pontic,BRG-Abutment. He was vey rude and did the work very recklessly and it was very painful, however he did not seem to care, he never issued a word just kept on working on the tooth area.
On 3/12/2014 Brian Wilkinson did the following - 06998 Seat Bridge, Adjust and I paid him $300.00 of the $794.00, for a balance of $449.00.

What if anything is being disputed? I can not go back to that animal - and complain, I don't feel safe in that office environment, while the dentist and his office girl were working on my tooth all they were doing were complaining about the other people employed at that dental office

However I have a very bad situation. I cannot brush or floss between my front teeth were he placed the bridge, it does not fit properly, and it looks unprofessional and I'm afraid to simile.
Simply stated I want all my money back so I can go to another professional dentist and have the work completed in a professional and satisfactory way."
