Statement of Claim
"I went to this hospital for altered mental state if you read the medications administered you can see that my leg was broken in their care in which I clearly remember them doing along with the other mental abuse and complete disregard to my life, under the assumption that I was a homeless drug addict in which I am not. I was taking a weight loss drug in which I disclosed. There is no reason I would be given meds for deep vein thrombosis or morphine or hydrocodone. No reason I was deprived water or tied like a animal given a cat scan which I wasn't or said to give 6 different forms of stool softner. Or lied to after the hospital told my boyfriend I had to remain off my feet due to the medication but fall repeatedly and be denied proper medical assistance at a the Bedford hospital due to the lies of the Arlington hospital trying to cover up the fact they tried to kill me with meds after sedating me with morphine Haldol hydrocodone and a series of drugs without food or water besides what my boyfriend gave me they should fire these people probably pressing charges I have called their offices repeatedly no return call.. This is sick sick sick....and I was sedated to b abused and given a bunch of unnecessary test to cover up the broken leg.....Thank you Dr. Borelli, Nurse Hillary Cummings, and Dr. Carlos Morales Who actually found me unresponsive in the hospital thanks to Dr. Borelli and his side kick nurse."

  • 04-19-2016 — Arlington Memorial patient brought by paramedics not combative restrained or delusional upon arriving just failed to recall the date after taking Adipex or fen fen and lack of sleep which was develuged to the staff upon arriving

  • - On more than one occasion the hospital has directly provided Ms. Pollefeyt with information requested, and have privately advised her that it cannot substantiate her allegations.
    - The hospital will cooperate with any law enforcement investigation of this matter.
    - Further, Texas law provides a specific process by which malpractice claims are handled, and the hospital believes that is the proper forum for Ms. Pollefeyt’s allegations and any further response by the hospital.
    - Finally, while respecting that Ms. Pollefeyt has chosen this forum, the hospital does not believe it is appropriate to discuss protected and private health information in this setting.
    - Letter previously mailed to Ms. Pollefeyt is attached.

  • I was contacted by the hospitals Attorney today and will have a attorney get resolution, because It's getting sorta hairy and I don't know the law well enough yet to try to deal with these people
