Statement of Claim
Claimant says:
"Unfortunately my father recently passed away on Monday, 7/20/2020 at the Kindred Hospital South Bay.  At 89 years old, he used to be a very healthy person living in a nursing home in Temple City for about 1.5 years. Since the lock down of all nursing homes around March 16th, the staff there did not feed him well and he's become malnourished. As a result, the primary physician was supposed to arrange to perform a G-TUBE procedure at the Methodist Hospital in Arcadia. Unfortunately my father was Covid-19 tested POSITIVE throughout the waiting processes and all surgery was suddenly being put on hold by the doctors.

On May 16th, without fully explaining the reason or disclosure of both Methodist Hospital and Kindred Hospital's ability to perform simple J-TUBE procedure, Methodist Hospital gave us no choice but transferred my father to Kindred Hospital South Bay in Gardena due to "healthcare policies", etc for long term care. Since then, my father has been losing weight without good care. Kindred's pharmacist ULYSSES YEE even apologized to me that they somehow recorded wrong info about father's weight, not discovering my father has been losing weight sooner!

After repeated Covid-19 tested positive several times without any symptoms whatsoever, at last he was being tested NEGATIVE on 6/26 and was preparing for J-TUBE procedure at the Torrance Memorial Hospital in the week of JULY 6th.  Later on we found out that Kindred Hospital was not qualified to perform J-TUBE surgery. Knowing my father has been o TPN for so long without proper nutrition, this whole case management process should have been addressed much earlier without much delay!

Unfortunately for whatever reasons, my father caught a new yeast infection right in the middle of transferring him from the COVID section to the Non-COVID section of the rooms in the week of June 28th. I suddenly got a call from the hospital that my father had a breathing problem and later was confirmed that he had a yeast infection on his lungs. He was immediately transferred to the ICU on 7/4, intubated and with a chest tube to drain fluids out of his lungs. The hospital could not explain the root cause of this new infection and suspected that it could be the PICC line used for the TPN solutions. 

Since 7/4, my father has been treated with all sort of antibiotics and finally the FiO2 recovered to 50% with 100% oxygen saturation level. He never had any heart related problems. I called the hospital to check my father's conditions twice every day and ensure he's stable. On Monday AM, 7/20, I suddenly got a call to meet at the hospital. After 30 minutes of Code Blue procedures, the ICU staff told me that my father's heart just stopped even though the night before he's been stable. We have many reasons (emails, photos, videos) to believe that my father was not being treated properly before and after he was being transferred from COVID- to NON-COVID section and while he's at the ICU."
