Statement of Claim
Claimant says:
"I tried to ask Mr. Chris Lee not to take taxes off the top before giving pay. I also tried to tell Mr. Chris Lee that he can not use my socail security number to file me on taxes an take the taxes out of my pay off top including my last check. Mr. Chris Lee told me go to court that he will continue to take taxes out. Mr. Lee also told Evanston Police that he will take tax out my next check. I tried asking for my right pay 8.25 instead of 6.25. Then Mr. Chris Lee fired me after I handed The 2 week notice."

1. Recovery of Losses: supposed to be 169.13 paid to me, remaining money still owed from (143.86 check) that was given for,20&half hrs. Plus 2 days scheduled 12-29-2010(-)01-01-2011 21hrs for 173.25 Dec 29, 2010 $198.52
2. Plus the pay for the two weeks notice that I gave and was fired right after handing to Mr. Chris Lee for not turning my socail security number in and taking out taxes. (for two weeks notice add)normally work 3day a week for 21hours Jan 31, 2011 $643.50
