Statement of Claim
Claimant says:
"I was charged a "hospital service" fee two months after I went to a doctor appointment. The appointment was with an ear-nose-throat doctor for ear infections. The doctor clinic was located in the "doctor offices" section in a different, but connected building next to the hospital. I paid a $40 co-payment at the time of service, so I called and asked for an explanation of the new charges. My three phone calls were never returned. I had my insurance call and was told by my insurance that they said it was for "observation." They said "something" (not detailed in record) was removed from my ear. He used a vacuum to suck out wax and a little puss from a pimple. If I had gone to any other doctor's clinic, my co-pay would have covered it. I am being charged $92.80 in addition to the $40.00 co-pay. Possibility of extra charges was not disclosed prior to service."
