Statement of Claim
Claimant says:
"On Oct. 13, 2015 I attended a meeting at Universal Men's Clinic, located at 310 N. Wilmot Rd., Suite 210. The purpose of the meeting was for me to gain information concerning the ED/PE Medical Program which they had been advertising. The fee for the consultation was in the amount of $199.00 which I agreed to and paid in full. That was acceptable to me and is not related to the purpose of my complaint. In addition to the meeting, I later agreed to enroll in one of their ED treatment programs for a one year period at a cost of $1,329.00, to be paid with a down payment of $200.00 and 9 monthly payments beginning 11/15/2015 in the amount of $125.33. I subsequently made the first payment, as agreed, for a total of $325.33 as having been applied towards the contract. At a later date and after consulting with my personal Urologist, I discovered that I no longer needed the services of Universal Men's Clinic, and made an appointment to cancel the contract. On December 3, 2015, I met with Counselor Hank Schemper to cancel the contract and receive a partial refund of the monies which I had expended as of that time. I was told at that time that I would receive a refund in the amount of $111.15. Instead of receiving that amount, I received a check in the amount of $47.24 dated 12/3/2015. I called Mr. Schemper and discussed the situation with him, and he referred me to their office manager, a man named Roberto (?). After numerous subsequent attempts to contact this person, I never received any return calls for any type of explanation. At this point, I feel that I should be entitled to the return of my $200.00 deposit less the $47.24 for a total of $152.76. I subsequently discovered that certain information had not been sent to me as agreed, and for that reason, I feel that they were in violation of the original contract."
