Statement of Claim
Claimant says:
"This company somehow was able to get my visa card number without my consent. I was somehow enrolled into their dental plan. I never heard of this company, and I was surprise when I checked my Visa card activities and saw that $16.50 was taken from my account on 04/07/2012. I saw a phone number next to Multifex Dental. I called the number and was put on hold, and another time I was disconnected. I also left a message. Not one person returned my call. In May they sent me a big package with papers explaining what Multiflex was all about and what was included in their plan for dental coverage. Once again I called , no response. On 06/09/2012 $ 17.48 was taken from my visa account. I called and I finally got a live person . I told him I never enrolled in the dental program, and I didn't want or need their plan, and I was informed by him that he would cancell the plan and returned my money which was $33.98. This person sent me a confirmation about the cancellation of the dental plan. But on 08/08/2012 this company took $17.48 again, after I was informed that this policy was cancelled. I called every day since 08/08/2012 but i was put on hold and all other times I just got recorded messages, which told me to leave a message and phone number. This company got my name and other info off the internet. HOW? I DON'T KNOW."

  • According to our records, we received Ms Clarke's application that was filled out on February 22, 2012. The application shows a request for a March 1st effective date. All the information provided to us by our aggregator, along with her chosen method of payment, was submitted on the aforementioned application. At the bottom of the application she acknowledged the terms with her “esignature” next to a disclaimer that authorizes Nationwide Muliflex Dental Plan to deduct payments from her Visa card. There is no possible way that we would be able to obtain her financial or personal information without her providing it to our aggregator. We have only collected 3 payments from her since she signed up with us and technically she owes three additional payments as she did not terminate until August but we have since closed her account as of May 31, 2012. As of now, her account is closed and she owes us no further premiums and we will not attempt to collect. There will be no monies refunded as we were authorized to deduct her monthly premiums and did so in good standing.
