Claimant's opinion: I had an issue that resulted in my being unable to work for 3 weeks, going to the ER, Urgent Care, family doctor, and a surgical consult. I have short-term disability available through my job. For the surgical consult appointment, Dr Joesph Pogorelec, and his nurse Tanya Koziel failed to completely and accurately document what I stated when questions were asked. The result was that their answers left the interpretation wide open, insinuating I was 'cured' when in fact I was not. A request to provide clarification was denied.What was written: "Patient states her discomfort has improved significantly." What was left out: "...compared to Thursday when I was in the ER."What was written: "She is able to sit down now."What was left out: "....for about 2-3 hours before pain resumes, and only able to sit on my hip. I cannot sit properly yet, much less do so for 10 hours at a desk.What was written: "Patient rates her pain minimal to a 3 out of 10."What was left out: "...at this moment. Earlier this morning it was more like a 6, and after sitting a few hours it increases to 6-8 out of 10.Due to a lack of complete documentation in my medical file, and refusal to provide a clarifying addendum, my short-term disability leave was denied as it was interpreted that at my surgical consult (not bad enough for surgery, and best to just wait out the symptoms to improve), I had no reason for missing the subsequent days at work until I could sit at my desk for 10 hours a day without pain as the doctor notes indicate "I was better". Basically, they got their money so who cares about the repercussions of how words are interpreted in their case notes as it's not their issue. 10 minute drive for subpar care vs 1 hour drive for doctors who care about you as a person and willing to help you out as best they can; I will drive to Madison to see a doctor going forward as all my trust in the doctors at Divine Savior in Portage is gone.
