Claimant says: "Thursday April 26, 2012 I (Donna Howard) went to have a tooth extracted. At that time I paid for the procedure in full $300.00. At the time of the procedure the dentist was told what I was allergic to (Lydocain) he said, if we give you nitrous it should calm you down, so that's what he did, then he administered LYDOCAIN, the very thing I said i was allergic to after the surgery I was sent home with NO POST CARE instructions.
