Claimant says: "I visited this medical office for a consultation on a facial skin blemish. Reception asked for insurance card and photo copied both front and back. Reception then asked for a co-pay which I paid. Physician Assistant examined blemish and advised that it could be removed but sutures would be required and the remaining scar would probably be more noticeable than the blemish.
