Claimant says: "On November 17, 2011 I was charge I was charge $70.00 co-pay in error. My dental insurance plan paid Cherry Hill Family Dental on 12-27-11 $70.00 also. They agreed that I was overcharged and would send me a check for the $70.00. When I call the manager Jon told me it is the Corporate Office that will send it to me. They will not tell me the name of the corporate Office or telephone number. Bill was paid with my Visa card and I asked them to credit the card. They would not do that either.
