Claimant says:
"My name is ****. I initially went to what I thought was called "Men's Health" for a personal problem with ED. I paid them $1000.00. I started receiving calls that I didn't pay the payments of the remainder of the charges. I never received an invoice. I am thinking I need to get an attorney as when they sent me a copy of the contract that they said I signed, I found that my name was forged! It is NOT my signature. I would appreciate any assistance that you can give me as I don't feel that I owe anything as they did forge my signature."

"The owner Mark Johnson is declaring bankruptcy to avoid creditors he and business partner, Taylor Cloys Ward defrauded under evolving ED Clinic names, THE YALE CLINIC LLC, Synergy Health Centers, Gladiator, Guardian, Men's Health LTD, and LLC to name a few."

"This is all part of a scheme of unscrupulous telemarketers using multiple ED clinic names, some registered and some not, posing as doctors and hospital administrators. Some are posing as doctors or paying local doctor(s) to use their licenses to be in business as a urologist, but what they really are are telemarketers in pharmaceuticals sales."
