Claimant says:
"Received a letter stating my insurance was cancelled for non payment. Said I did not send in my November payment. This was on 12/12/2011. Insurance was termination date 10/31/11.
Looked up my bank statement and found cancelled check and went back to July and had payments from July - November 2011.
December payment had already been made but had not cleared.
I send a coupon every month stating the due date and now they are telling me that if it says it is due on November 1st. that is for Oct. payment.
They have no phone number that you can contact Pro View Cobra directly.
They have said that the insurance has been reinstated but it is a month behind, Dec. payment is for November.
I get 752.00 a month Social Security and pay them close to five hundred a month, and now they are telling me I am a month behind.
I have not received my coupons for 2012 and they are now saying I don't need a coupon to mail in my payment. I refuse to send them a payment without a coupon, they can't keep it straight with a coupon."
