Statement of Claim
Claimant says:
"To whom it may concern: I was a patient at Avamere of Hillsboro Oregon for about two months and while I was at their facilities a wheelchair was rented from Apria Health Care, for me. When I was release from Avamere, I was sent to another rehab center in Arizona. I was not allowed to take the wheelchair because it was a rental, this statement came from Avamere staff, and I was told that Apria would be contacted for pick up. When Apria went to Avamere they were told they didn’t know where the wheelchair was. Every time Apria went to Avamere they were told the same thing. I knew nothing of what was going on because I was in hospitals and rehab centers in Arizona, until October 2016. By the time I found out what was going on, the account had been turn over for collections, with Global Receivables Solutions. I contacted Avamere and spoke with Rob, the Administer, and was told he would do a sweep to look for the wheelchair and would call me back the next day. When he didn’t call me back I called him back with more information on the chair with a request for call back. I did receive a call back from Rob’s co-workers whom advised me Rob was out of town. I called back on his scheduled return and left another massage, still no return call, I also called another of Avamere Administers Wesley Baum, whom I also explained the problem too and was told he would consider the matter and call me back, again this didn’t happen. I called Mr. Baum several times and left messages, requesting return calls, but it didn’t happen. On the one and only time I talked to Mr. Baum, he told me he had done a sweep of the wheelchairs and they all had Avamere stickers on them but he didn’t check serial numbers or model numbers. On the day, I left there, I was taken to the airport and put on a plane to Arizona, where I was put into another hospital and then another rehab center until October of 2016, both the hospital and the rehab center will tell you I arrived without a wheelchair. Please understand since my stroke I’ve had to retire from my job and now live on a fixed income, I can’t pay $730.00, for a wheelchair I don’t have. I also have a witness to this conversation her name is Karen Tabla, 480-396-8024, she was my caregiver at the time, please contact her if additional verification is needed. When I was contacted by Global Collections I was told that I had signed for the wheelchair so I am reasonable and I had to pay. I tried to tell them I don’t know what I signed, I was to drugged up to even know what day it was, let alone what I was signing. I’m going to contact the BBB in the state of Oregon, hopefully, they can help me.

  • 1 sent both of my sons your telephone number along with Karen Tabla. Karen was my caregiver at the time she will testify to the fact that I was not allowed to take the wheelchair. My son Chad will testify that I arrived in Arizona without a wheelchair and my son Clint will also testify that I was not allowed to take the wheelchair because he had to go in the airport to bring me a wheelchair.

  • Everything I have said in my complaint is true, but what is sad is Avamere, allowed Apria Health Care, (this is where the wheelchair came from) to come into my room at 10 at night, and made me sign for this wheelchair. I was on so much medication I have no idea what I was signing. Apria Health Care, should be ashamed, of their practices. I was not aware of what was going on (being sent to collections) because I was in hospitals and rehab centers from April 29, 2016, to Oct 30, 2016, so I didn’t receive any mail. I do have several people who can testify to these facts.

  • On 6/16/17 I went forward with this claim. I filed a claim with BBB of Oregon and also check on filing a claim with the small claims court which will be easier then I expected even though I live in AZ now,because I still have lots of family in Oregon including the witness in this case

  • Thank you for your response and I assure you everything I've said is true. I have three witness  that will tell the same story but, Avamere, will not return calls.

    Wheelchair was left in Avamere Rehab's custody. Avamere Rehab should have a protocol in place for handling equipment returns securely. Avamere should IMO pay Apria's $730 bill. More...
